With the crucial exception of a couple of quite important and influential observers the elbow was obvious to everybody inside the ground.
It wasn't a stamp, it was a stumble. He tripped backwards over winnall. The angle is just poor on the video so it looks like a stamp
I think that I would give O'Neil the benefit of the doubt there. I think it was an accidental stumble and it did look as though it was O'neil's studs which re-arranged Sam's features, but still think it looked accidental. Didn't see the elbow, but you are probably right and that is why he was on the floor in the first place.
Looked obvious even from the West, I was looking direct at it. Couldn't believe the ref and assistant missed it, all 3 officials were shockingly bad all game.
I don't know what happened I wasn't looking at the time an I can't tell from the replays but correct me if I'm wrong, if violent conduct goes unpunished during a game, isn't it eligible for retrospective review and subsequent action? Wouldn't we have heard something by now if anything was being done about it? If it's an obvious elbow (never mind the deliberate stamp some people mentioned) then why haven't we heard about retrospective action?
